Criteria for Eligibility

For a nurse to be eligible for VDAP, the following criteria must be met.
  1. Active license as registered nurse or licensed practical nurse in Alabama, or hold a valid temporary permit to practice as an RN or LPN in Alabama;

  2. Admit to a substance use disorder, or a mental or physical condition that can or does cause impairment in the workplace;

  3. Voluntarily request to participate in VDAP

  4. Agree to cease nursing practice immediately and not return to practice until treatment provider and VDAP staff agree the nurse is safe to practice (and the nurse has signed the VDAP Agreement for monitoring);

  5. Have had NO disciplinary action in any state or jurisdiction;

  6. Have had NO nursing practice problems resulting in patient harm or death
    Provide all needed information and all releases required to obtain any information requested;

  7. Acknowledge, in writing, extent of disease process and its effect on your nursing practice, and agree, in writing, to all terms in VDAP Agreement.
