Substance Abuse Treatment Providers

REQUIREMENTS FOR CHEMICAL DEPENDENCY TREATMENT PROVIDERS

1. All nurses (LPN, RN, BSN, CRNA, CRNP, CNM, CNS) in the program must self report to ABN within 72 hours of admission if not already Board referred or involved.
2. A licensed physician (MD/ DO) must make axis I – V diagnoses.  
3. The program must randomly drug test nurse-clients throughout the  treatment stay.
4. The treatment duration must be a minimum of 6 weeks.
5. Aftercare or continuing care should be at least 12 months in duration.
6. Provider should contact ABN prior to or within 48 hours of admission to request Board records.
7. ABN should be notified immediately if nurse leaves treatment AMA or is required to leave the program prior to completion.
8. A complete, detailed biopsychosocial assessment and a thorough   discharge summary should be mailed or faxed to ABN within 10 working days of admission and discharge respectively.
