You must submit a request for an “Actively Deployed Application” with a copy of the military orders which were in place when your license lapsed no later than 60 days of return from deployment.
Based on the orders you submit, if you are eligible for the Actively Deployed Application, one will be sent to you in the manner you request: email, mail, or fax.
