Please carefully review ALL information if you plan to apply for reinstatement of your revoked nursing license. Incomplete applications are not considered. It is the applicant’s responsibility to ensure that a complete application is submitted to the Board.

ALL Applications/Applicants must:

  1. Be submitted no earlier than ONE year since revocation.
  2. Be a citizen or legal US resident. For information on meeting this requirement, please refer to the Citizenship/Legal Presence FAQs.
  3. Demonstrate good moral character. There are Regulatory Questions on the application.
  4. Be complete. A complete application often includes the following:
