Monthly Self-Report
The deadline for submitting the November 2026 monthly self-report has been extended from November 5, 2026, to November 13, 2026.
Please Note: When completing your November 2026 self-report, please report information for October 2026.
All participants must complete the paper self-report using the Self-Report Form below and submit it directly to ABN by November 13, 2026.

Quarterly Employer and Counselor Reports
Additional instructions regarding quarterly employer, aftercare, and counselor reports will be posted on the ABN website before the reports are due in January 2027. Please check the website for updates.
Drug Screening
The Board's random drug screening program remains in effect. All VDAP and Probation participants who are subject to drug screening must continue checking in with Affinity and complete drug screens when selected.
