Alabama Board of Nursing Single State to Multistate Application Licensure Application

Instructions: Please submit all requirements

  • You must have an active Alabama single-state license. If you previously held an Alabama nursing license/permit and your license/permit is lapsed, revoked, or retired, you must apply for and must have your license reinstated, this is not correct application to have your license reinstated.
  •  CE: You must attest to having earned twenty-four (24) hours of CE within the twenty-four (24) months before submitting the license application. This may be prorated based on the date of issuance of your license.
  •  APPLICATION FEE $200: personal check (name of applicant imprinted on the check, drawn on in state bank), business, cashier check or money order. Please ensure the money orders are completed with your signature. Payable to ABN.
  •  Fingerprinting Required: You must have an application on file with the Board of Nursing prior to submitting fingerprinting.
