Online services, including monthly self-reports, quarterly employer reports, quarterly aftercare reports, and quarterly counselor reports, remain unavailable. While we continue to work to restore these services, please take care to follow the instructions below.
The October 5, 2026, monthly self-report deadline and the October 10, 2026, quarterly report deadline have been extended to October 31, 2026.
What do you need to do:
- Self-Reports: Complete the paper self-report using the Self-Report Form below and submit it directly to ABN by October 31, 2026.
- Self-Report Form
- VDAP participants: Send your self-reports to VDAP@abn.alabama.gov.
- Probation participants: Send your self-reports to Probation@abn.alabama.gov.
- Quarterly Reports: Provide the appropriate paper report below to your employer, aftercare provider, and/or counselor, as applicable, for completion. The provider or employer must send the completed report directly to the ABN by October 31, 2026.
